Insurance
I do not currently take insurance, however you will be provided with a receipt/invoice that may be submitted to your insurance carrier for reimbursement. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

What is my health insurance coverage and benefits?
What is my deductible and has it been met?

Payment
Cash, check and all major credit cards are accepted for payment at time of appointment

I appreciate the cost of comprehensive treatment and work with patients to make your treatment affordable. My fee schedule for initial evaluation, usually 60 minutes is $300. Follow up visits, usually 30 minutes is $150. I employ a sliding scale fee schedule for established patients, to prevent the cost of treatment to become an obstacle to treatment.

The fee schedule for the New York State Medical Marijuana program initial evaluation is $300 and follow up visits are scheduled annually.
